• Also list home remedies, nonprescription drugs, birth control pills, mineral or herbal supplements, vitamins
• Better ask patients to bring all the medications

• Specific reactions to medications (rash or nausea)
• Allergies to foods, insects or environmental factors

Tobacco use

• Cigarettes are reported in pack-years
• Multiply the number of packs of cigarettes smoked per day by the number of years the person has smoked
• For example: 1 pack-year is smoking 1 pack per day for 1 year
• If someone has quit , note for how long
