For an adult that is mentally or physically disabled and not capable of protecting him or herself,
contact the Adult Protective Services office before any disposition is made for the patient (before
being released).

All cases involving minors (less than 18 years of age) require immediate notification
to the Child Protection number (1-800-752-6200).

All information about referrals and involvement of outside agencies must be documented in the
patient’s medical record.
Sexual Assault: Reporting and Treatment Process
1.
The responsibility for notification lies with the hospital staff member who learns of the incident.
The House Administrator is notified at the time the incident is recognized.
2.
A Suspected Abuse/Neglect Reporting form (MR-512) must be completed in its entirety on any
patient (
child or dependent adult
) suspected to be a victim. The original copy is placed in the
patient's medical record. The second copy is to be sent to the Case Management Coordinator.
