  • The major advantage of helical scanning compared to the traditional shoot-and-step approach, is speed; a large volume can be covered in 20-60 seconds. This is advantageous for a number or reasons:

    • often the patient can hold their breath for the entire study, reducing motion artifacts,
    • it allows for more optimal use of intravenous contrast enhancement, and
    • the study is quicker than the equivalent conventional CT permitting the use of higher resolution acquisitions in the same study time.

It may also improve the characterization of tissues allowing better tumor differentiation.

  • The half-life of 99Mo is much longer than that of 99mTc. 50% of equilibrium activity is reached within one daughter half-life. Hence, removing the daughter nuclide (elution process) from the generator ("milking" the cow) is reasonably done every 6 hours or, at most, twice daily in a 99Mo/99mTc generator. Most commercial 99Mo/99mTc generators use column chromatography, in which 99Mo is adsorbed onto acid alumina (Al2O3). Pulling normal saline solution through the column of immobilized 99Mo elutes the soluble 99mTc, resulting in a saline solution containing the 99mTc which is then added to an appropriate concentration to the organ-specific pharmaceutical to be used. The isotope can also be used without pharmaceutical tagging for specific procedures requiring only the 99mTc as the primary radiopharmaceutical. The useful life of a 99Mo/99mTc generator is about 3 parent half lives, or approximately one week. Hence, any clinical nuclear medicine units purchase at least one such generator per week or order several in a staggered fashion.

  • 99Mo can be obtained by the n activation (n,γ) of 98Mo in a high neutron flux reactor but the most used method (>95%) requires a uranium target with high enriched uranium (up to 90% 235U) or low enriched uranium (less than 20% 235U). The target should be irradiated with neutrons to form 99Mo as a fission product (6.1%).

Druhy scintigrafie Než se budeme zabývat konkrétními fyzikálně-elektronickými metodami pro realizaci scintigrafického zobrazení, stručně si uvedeme rozdělení (klasifikaci, kategorizaci) scintigrafických metod. Z hlediska časového lze scintigrafii rozdělit na dva druhy:

  • Druhy scintigrafie Než se budeme zabývat konkrétními fyzikálně-elektronickými metodami pro realizaci scintigrafického zobrazení, stručně si uvedeme rozdělení (klasifikaci, kategorizaci) scintigrafických metod. Z hlediska časového lze scintigrafii rozdělit na dva druhy:

  • Statická scintigrafie je základním druhem scintigrafie - je to prostě jeden či několik scintigrafických obrazů vyšetřované oblasti (bez ohledu na čas). Buď se snímá totéž místo z různých projekcí (AP, PA, DX a pod.), nebo několik různých míst organismu - takové scintigrafie se někdy nazývají též multistatické.

  • Dynamická scintigrafie Sledujeme-li pomocí radioindikátoru děj měnící se s časem a zajímá nás jeho dynamika, provádíme dynamickou scintigrafii - je to série (statických) snímků vyšetřované oblasti, snímaných postupně v různých časech. Rozdíl mezi statickou a dynamickou scintigrafií je analogický jako mezi fotografováním a filmováním: filmový záběr se skládá z velkého počtu krátkých (statických) snímků (políček filmu) v rychlém sledu za sebou, které pak při rychlém promítnutí vzbuzují dojem plynulých pohybů. U dynamické scintigrafie můžeme nejen vizuálně sledovat pohyb a časové změny distribuce radioindikátoru v organismu, ale vytvářet příslušné dynamické křivky a matematickou analýzou stanovovat kvantitativní parametry funkce jednotlivých orgánů.

  • Z hlediska prostorového (geometrického) můžeme scintigrafii rozdělit opět do dvou kategorií:

  • Planární scintigrafie je základním druhem scintigrafického zobrazení - je to obraz projekce distribuce radioindikátoru v záření g do dvojrozměrné zobrazované roviny.

  • Tomografická scintigrafie poskytující prostorové trojrozměrné zobrazení. č SPECT Tomografická scintigrafie SPECT (Single Photon Emission Copmputerized Tomography - jednofotonová emisní počítačová tomografie) je realizována jako série planárních obrazů vyšetřovaného místa, snímaných pod mnoha různými úhly (0o-360o) detektorem kamery obíhajícím kolem pacienta. Počítačovou rekonstrukcí se pak z těchto obrazů konstruují tomografické obrazy příčných řezů vyšetřovaným objektem. Série těchto obrazů transverzálních řezů pak vytváří celkový trojrozměrný obraz distribuce radioindikátoru. č PET Další tomografickou metodou je PET - pozitronová emisní tomografie. Zde je aplikován pozitronový b+ radioindikátor, který v místech své distribuce emituje pozitrony e+, které vzápětí anihilují s elektrony e- (e++ e- č 2 g) za vzniku dvou fotonů g vylétajících do opačných směrů (180o). Tomografického efektu se pak dosahuje současnou koincidenční detekcí těchto dvojic fotonů g, načež počítačovou rekonstrukcí velkého počtu takových koicidenčních paprsků se opět vytváří tomografický obraz příčného řezu vyšetřovanou oblastí. Principy tomografické scintigrafie budou podrobněji rozebírány v §2.3.



Vznik denzitního obrazu Má-li podle obr.3.2.3 vlevo svazek X-záření emitovaný rentgenkou a dopadající na vyšetřovanou oblast počáteční intenzitu (tok fotonů za 1s) Io, pak jeho intenzita I po průchodu tkání bude I = Io.e-Sm(i,j).Dx, kde m(i,j) je lineární součinitel zeslabení X-záření pronikajícího místem tkáně o souřadnicích i,j a Dx je velikost (délka ve směru paprsku) elementu tkáně. Hodnoty koeficientů m(i,j) závisí na lokální hustotě a protonovém čísle jednotlivých míst (i,j) tkáně. Logaritmováním se tento vztah dá upravit na tvar: ln(I/Io) = Sm(i,j).Dx, který říká, že logaritmus poměru intenzit X-záření vstupujícího do vyšetřované tkáně a z ní vystupujícího, se rovná součtu součinů lineárních součinitelů zeslabení a drah, které fotony X-záření v jednotlivých místech tkáně překonávají. Měřením při různých polohách rentgenky a detektoru se získá řada hodnot zeslabovacího poměru ln(I/Io). Počítač pak v zásadě řeší soustavu lineárních rovnic shora uvedeného tvaru, čímž se získají hodnoty lineárních součinitelů zeslabení X-záření tkáňových elementů v jednotlivých místech (i,j) tkáně - vzniká obraz denzity tkáně. V praxi se nepostupuje výše uvedeným přímočarým způsobem. Výsledný transverzální CT obraz se získývává rekonstrukcí z jednorozměrných profilů distribuce intenzity prošlého paprsku X-záření při otáčení rentgenky a protilehlých detektorů kolem vyšetřovaného objektu. Pro tuto rekostrukci se používá většinou metoda filtrované zpětné projekce, někdy i dokonalejší (avšak výpočetně náročnější) metoda iterativní rekonstrukce. Tyto rekonstrukční metody, které jsou anologické jako u SPECT, jsou stručně popsány v §4.3 "Tomografická scintigrafie". Denzita vyšetřované tkáně se většinou porovnává s densitou vody a v obraze CT je číselně prezentována v tzv. Hounsfieldových jednotkách HU = 1000.(mtkáň - mvoda)/mvoda , zavedených předním průkopníkem v oblasti CT G.N.Hounstfieldem, spolu s A.L.Cormackem.
