Admission

Eve Everywoman, a 60-year-old female, has been diagnosed with follicular thyroid cancer. Dr. Patricia Primary has recommended surgery to remove Ms. Everywoman’s thyroid and post-operative radioactive iodine treatments. Dr. Primary’s office has called the Good Health Hospital and scheduled an inpatient admission for Ms. Everywoman’s total thyroidectomy. Ms. Everywoman presented at Good Health Hospital to the admitting department on the morning of her surgery. Alice, the admitting clerk admitted Ms. Everywoman for the surgical procedure. Dr. Cutter was Ms. Everywoman's attending physician. Alice, Admitting Clerk, accessed Ms. Everywoman's appointment for the inpatient encounter and noted that she had arrived to fulfill the scheduled two-day inpatient encounter. Alice noted that Ms. Everywoman was a patient of the surgical service and would be staying on the fourth floor, ward B, room 4020, bed A. Since Ms. Everywoman will require post-operative treatment, her endocrinologist Dr. Adrienne Allgland is entered as the consulting physician for the inpatient encounter.

Activate Order

In preparation for surgery, Dr. Cutter orders standard pre-surgical laboratory tests. He also orders serum TSH and ionized Calcium with standard frequency for the next two days. Since there is an implementation agreement between the ordering system and the laboratory system that all orders are assumed to be accepted by the laboratory system, the ordering system utilizes an order message that does not utilize an accept acknowledgement. Each laboratory order POLB_IN002121 is sent from the ordering system to the laboratory system. The laboratory system receives the order, and after processing the order determines that it meets the laboratory systems requirements for accepting an order. The laboratory system does not return a message to the ordering system accepting each order.

Abort Inpatient Encounter / Abort Laboratory Orders Pending Specimen Collection

Soon after admission one hour prior to beginning the first specimen collection procedure, Ms. Everywoman complains of fever. Erin Medsurg, staff nurse, confirms Ms. Everywoman has a high fever that would prevent her from having the surgery that day and notifies Dr. Cutter. Dr. Cutter authorizes to abort the inpatient encounter with a discharge disposition "Discontinued care." Ms. Everywoman is sent home to recover prior to the surgery. All active orders including laboratory orders pending specimen collection are aborted by the ordering system and sent to the laboratory system POLB_IN005150. The laboratory system receives each order abort, and after processing it determines that it meets the laboratory system’s requirements for aborting an order. The laboratory system returns a message for each order to the ordering system acknowledging the order abort POLB_IN005151.
