Acute

» ciprofloxacin
: 500 mg orally twice daily

» moxifloxacin
: 400 mg orally once daily
OR
» levofloxacin
: 750 mg orally once daily
OR
» doxycycline
: 100 mg orally twice daily
OR
» amoxicillin
: 1000 mg orally three times daily
Secondary options
» clindamycin
: 600 mg orally three times daily
» 
Oral quinolones (i.e., ciprofloxacin,
levofloxacin, moxifloxacin) and doxycycline are
considered equal first-line agents. Doxycycline
is usually reserved for patients with allergies
to quinolones. In the event of an allergy to a
first-line agent, clindamycin is an acceptable
alternative. Amoxicillin is an alternative
therapeutic option if the isolate is known to be
susceptible to penicillin.

In pregnant patients with mild naturally
occurring cutaneous infection, oral amoxicillin
may be used instead of ciprofloxacin.

In anthrax cases related to biological terrorism,
amoxicillin resistance should be suspected.

Once antibiotic susceptibilities are available,
therapy can be completed according to antibiotic
sensitivity.
» 
Naturally occurring cases of cutaneous anthrax
are generally mild, and treatment can cease
after 7 to 10 days. Sixty days of therapy is
recommended for infection due to an act of
biological warfare.
