Assessment tools
In order to meet the requirements of this standard, individuals who are not achieving their targets first need to be identified. BPAC NZ have provided an audit process designed to identify and document the needs of people not meeting their targets in order to step up treatment and track progress in general practice. The following indicators can be used to assess quality.

The proportion of people with diabetes with agreed, documented targets.
The proportion of people with diabetes achieving their agreed targets.
