TAVSİYELER

A)  Semptomatik olgularda enfeksiyon kendiliğinden  sonlanabileceğinden 12 hafta beklenmelidir.

B)   12 hafta sonunda HCV-RNA’sı negatifleşmeyen olgular pegile interferonla tedavi edilmelidir. 

C)  Genotip 1 ile enfekte olgularda ve viral yükü yüksek olanlarda tedaviye RBV eklenebilir. 

D)  HIV koinfeksiyonunda tedaviye RBV eklenmelidir. Tedavi süresi 24 hafta olmalıdır.

ÖZEL HASTA GRUPLARINDA TEDAVİ SEÇENEKLERİ 

1.  HBV ko-enfeksiyonu olan HCV hastalarında tedavi

  •  HCV mono-enfekte hasta gibi tedavi edilir, HBV replikasyonu belirgin ise tedaviye nükleozid/nükleo-

tid analogları eklenir.

2.  Hemodiyaliz hastalarında HCV tedavisi

  •  IFN monoterapisi ile doz ayarı yapılarak tedavi edilirler, RBV doz ayarı ile tedaviye eklenebilir. 

  •  Direkt etkili antiviraller ile ilgili veriler beklenmektedir.

3.  HIV ko-enfeksiyonlu hastalarda HCV tedavisi

  •  Endikasyonlar HCV mono-enfekte ve HIV ko-enfekte hastalarda aynıdır. Ancak, ilaç-ilaç etkileşimle-

rine bağlı olarak tedavi değişikliği veya doz ayarlamaları söz konusu olabilir. Bununla birlikte hasta-

ların tedaviyi kabul etmesi, ilaçlara tolerans ve kontrendikasyon durumları göz önünde bulundurul-

ması gereken faktörlerdir.

4.  Böbrek transplant hastalarında HCV tedavisi
